A case report of vitamin C supplementation in a 67-year-old man with leukopenia
White blood cell (WBC) count is one of the most routine laboratory tests in clinical practice, and values that are outside the standard range are often a cause for concern. Leukopenia is defined as a white blood cell count <4,000 cells/µL Neutropenia refers to an absolute neutrophil count <1,500 cells/µL. Neutrophils make up the largest share of the circulating supply Leukocytes and are responsible for absorbing pathogens, so a deficiency can weaken the immune system. Neutropenia may occur Autoimmune diseases, infections, medication side effects, deficiency of folate, vitamin B12 or copper or leukemiaamong other things. If the cause is unclear or the count is severely abnormal, a bone marrow biopsy is often ordered to check whether neutrophil reserves are sufficient.
One explanation that often goes unconsidered is benign ethnic neutropenia (BEN)where lower neutrophil counts reflect a genetic background rather than disease. On average, people of African descent have white blood cell counts 700 cells/µL lower than whites.
Around 4.5% of African Americans and as many as 50% of Africans wear them Duffy null phenotypewhich is associated with malaria resistance and neutrophil counts falling below the standard range without additional risk of infection. However a Current case report published in Integrative Medicine: A Clinician’s Journal followed a 67-year-old African-American man who had lived with leukopenia for more than 30 years and also suffered from neutropenia. Because of these findings, he had to have two bone marrow biopsies decades apart, both of which came back normal. He had never had the symptoms typically associated with a low white blood cell count, such as easy bruising, recurrent infections, fever or fatigue. In addition, he reported a healthy lifestyle, running several days per week, eating primarily whole plant foods, meditating regularly, taking no medications, and occasionally taking oral vitamin C when traveling or feeling a cold.
A routine blood test in November 2023 revealed that his white blood cell count was 1,800 cells/µL (reference range 4,000 to 11,000 cells/µL) and his neutrophil count was 730 cells/µL (reference range 1,500 to 8,000 cells/µL), with all other values of his complete blood count within range. His doctor referred him to a hematologist for another biopsy to rule out leukemia, which came back negative. The protocol began in March 2024 and combined two forms of vitamin C. The morning and afternoon portions consisted of vitamin C powder mixed with 10 ounces of water. A liposomal form was used before bed, which saved him from drinking large amounts of fluids at night. On days when he felt exhausted, a second serving was added in the evening. Over the course of a year, daily intake was between 5,000 and 7,000 mg, titrated to gastric tolerance.
Over the following thirteen months, blood tests were performed at multiple sites, and total white blood cell (WBC) counts and absolute neutrophil counts (ANC) increased at each visit:

- WBC 2,100 cells/µL
- ANC 1,000 cells/µL

- ANC 1,450 cells/µL
- April 2025:
- WBC 3,200 cells/µL
- ANC 2,134 cells/µL
At final assessment, his white blood cell count had almost doubled and was just below the standard range, while his neutrophil count had exceeded the limit and was well within the normal range. He continues to take 3,000 mg of vitamin C orally daily.
A dosage of this level raises legitimate questions about tolerability and absorption. The Tolerable upper intake level (UL) is set at 2 g for adults aged 19 and over National Institutes of Health (NIH) indicates that vitamin C has low toxicity and is not believed to cause serious side effects at high intakes. Caution should still be exercised in patients with a history of hemochromatosis or hyperoxaluria. Absorption is another consideration The efficiency drops significantly before these amounts and the excess passes into the urine once the plasma is saturated. The combination approach may have helped overcome this obstacle liposomal vitamin C results in higher circulating concentrations than unencapsulated forms.
Population differences in white blood cell counts shaped the decades-long study of this patient. If doctors do not know that lower levels are common in people of African descent, the assessment can go beyond the clinical picture and add unnecessary burdens such as additional testing, financial costs and absence from work.
As with any individual case report, these results reflect an individual and cannot be generalized without further research. Whether smaller portions would produce the same result is still unknown, and controlled studies are needed to clarify what role oral vitamin C might play in supporting healthy white blood cell production.
