Introduction
Kasa Nidanam is one of the important topics of Ayurveda dealing with causes, pathogenesis, signs and symptoms of Kasa which is a disease characterized by cough. Kasa is caused by the vitiation of Doṣhas which disturbs the normal functioning of the respiratory system, says Madhava Nidanam. Exposure to dust and smoke, improper dietary habits, excessive physical exertion, and suppression of natural urges may contribute to its manifestation. There are five types of Kasa, namely Vataja Kasa, Pittaja Kasa, Kaphaja Kasa, Ksataja Kasa and Ksayaja Kasa. Understanding Kasa Nidanam helps in identifying its causes, recognizing clinical features, and establishing an appropriate Ayurvedic approach to diagnosis and management.
Nidana (Etiological Factors) Of Kasa
(Sushruta Samhita, Uttara Tantra, Ch 52 shloka 4)
- Dhuma: Exposure to smoke.
- Rajas: Exposure to dust.
- Vyayama: Excessive physical exertion.
- Ruksanna-sevana: Consumption of dry foods.
- Vishamasana: Improper eating habits.
- Ksavathu-vega-dharana: Suppression of the natural urge to sneeze.
Samprapti (Pathogenesis)
(Sushruta Samhita, Uttara Tantra, Ch 52 shloka 5)
Doshas get vitiated due to the intake of Nidana factors, especially Prana Vayu and Udana Vayu. When disturbed they force the air up and suddenly expel it through the mouth, making a peculiar sound, like that of a broken bronze vessel. Acharya describes the clinical manifestation as Kasa.

(Sushruta Samhita, Uttara Tantra, Ch 52 shloka 8)
Vataja Kasa is caused due to aggravation of Vata Dosha and is characterized as a dry cough. The patient may experience pain in the chest, temples, head, abdomen and sides. Other symptoms are emaciated or thin appearance, diminished physical strength, weakness of voice, and diminished vitality. The cough is recurrent and may be accompanied by a hoarse or altered voice. The cough is generally dry, with little or no phlegm.
Pittaj Kasa Lakshana (Clinical Features)

Pittaja Kasa according to Madhava Nidanam is characterized by burning sensation in the chest, fever, dryness of the mouth, bitter taste in the mouth and excessive thirst. Due to aggravated Pitta, the patient may vomit yellowish and pungent material, experience burning sensation all over the body and may have a pale or yellowish look.
Kaphaja Kasa Lakshanas (Clinical Features)
(Sushruta Samhita, Uttara Tantra, Ch 52 shloka 10)
According to Madhava Nidanam, Kaphaja Kasa is marked by a productive cough associated with Kapha aggravation. The patient experiences a feeling of heaviness in the chest and expectorates abundant, thick, and tenacious sputum. Other characteristic features include sluggishness, reduced appetite, and an increased tendency to sleep.

As per the book titled ‘Madhava Nidana,’ the Kshatja Kasa disorder happens because of the trauma in the thoracic cavity which is closely linked to excessive sexual activities, lifting heavy weight, having to walk long distances, and engaging in battles. The patient initially suffers from dry cough and later on may also have blood in his cough. There will be extreme pain in the throat and chest with the similar feeling of needles piercing the chest. This pain can increase by touch and can be either piercing or splitting. Some other symptoms that may accompany the above-mentioned symptoms are pain in the joints, fever, exhaustion in breath, thirst, and hoarse voice.

Unusual eating patterns, the consumption of inappropriate food items, the carrying out of excessive sexual engagements, restraint of the body’s needs, sadness, and the dysfunction of the digestive process are, according to some research, factors that play a role in the disruption of the balance of the three Doṣas and the formation of Kshayaja Kasa.
Nidana Sevana → Agni Dushti → Tridosha Prakopa → Dhatu Kshaya → Kshayaja Kasa.

Sadhya–Asadhyata (Prognosis) Of Kasa

(Charaka Samhita, Chikitsa Sthana ch 18 shloka 29-30)
The term Kshayaja Kasa is termed dangerous, particularly when it occurs in an individual whose strength is considerably reduced, but it can also be considered treatable in a patient with good strength. The two types can at times even be cured when they are relatively new and if the four factors of treatment (Chatushpada) are present in the patient. Kasa occurring in the old population is largely referred to as Yapya in nature. The first three types of Kasa in the order of Vataja, Pittaja and Kaphaja, are regarded as Sadhya while the types that are Yapya need continuous treatment and adherence to Pathya.
Conclusion
Kasa Nidanam, mentioned in the Madhava Nidanam, gives a systematic knowledge about the Nidana, Purvarupa, Lakshana, Samprapti, and Sadhya-Asadhyata of Kasa Roga. The text divides Kasa into five types: Vataja, Pittaja, Kaphaja, Kshataja, and Kshayaja, with each type having different clinical features depending on the type of Doshas and Dhatus involved. Thus, a sound understanding of its causative factors and how the disease occurs in the body is necessary to make an accurate diagnosis and plan the treatment. Therefore, the traditional description of Kasa in Madhava Nidanam is useful in deciphering the Ayurvedic method of determining the disease.
